Havrix / Vaqta
I want to be vaccinated to avoid getting hepatitis A and spreading it to others.
I am traveling to countries that have a high rate of hepatitis A.
Engerix B
I want to be vaccinated to avoid getting hepatitis B.
I was vaccinated with hepatitis B vaccine in the past, but never completed the full 3-dose series.
I travel outside the U.S.
Twinrix
I want to be vaccinated to avoid getting hepatitis A and B.

I am a male between the ages of 9-26 and want to prevent genital warts.
I am a female between the ages of 9-26 and want to prevent cervical cancer.
I want to be vaccinated to avoid 4 different types of HPV (2 types of HPV that account for 75% of cervical cancers cases!)
Boostrix
I am 18 years or older and I did not receive a vaccine for pertussis (whooping cough).
My last tetanusshot was 10 years ago or longer.
I have or will have close contact with a child younger than the age of 12 months and have not had a pertussis (whooping cough) - containing vaccine as an adolescent or adult.
Boostrix vaccines is used to help prevent diphtheria, tetanus, and pertussis in people who are between 10-64 years old. (TDap)
Pneumovax
I am 18 years old or older and have asthma, diabetes, or any other chronis medical conditions.
I am 65 years or older and I have never had a pneumococcalshot.
I am age 65 or older and had one pneumococcal shot, but it has been 5 years or more since that shot.
A vaccine to prevent bacteria that are a major cause of ear infections, strep throat, and serious pneumonia.
Zostavax
I am an adult ago 50 or older and haven't hada shinglesshot.
The same virus that causes chickenpox stays in your body and can later resurface as shingles. Prevent it by getting this vaccine.
Influenza Vaccine
I'd like to be vaccinated to avoid getting influenzaor spreading it to others this season.
I live with or provide care for a child younger than age 5.
I am or will be pregnant during the influenza season.
